Payments are due at the time that services are rendered.
We accept cash, debit, HSA cards, and major credit cards. ALL patients are required to have a credit card on file.
To cancel or reschedule your appointment, please call at least 24 hours IN ADVANCE of your appointment time. Failure to cancel your appointment 24 hours in advance will result in a $35 late cancellation fee.
We accept Aetna, Blue Cross & Blue Shield, New Directions, All Kids, Medicaid, Behavioral Health Systems, Cigna, Magellan, United Healthcare, Alabama Premier Group, Cypress Benefit Administrators, American Behavioral, Multi-Plan, Viva, First Health Network & Optum.
No Show Policy
If you do not call contact the office at all and do not show up for your appointment, you will incur a NO SHOW fee of $50.
Accepting clients from ESPYR, Magellan, American Behavioral, WorkPlace Options, Cascade, Mutual of Omaha, Behavioral Health Systems, New Directions, and Three Rivers.
Before turning accounts over to collections reasonable notice will be given including the opportunity to discuss a payment plan. I do reserve the right to use legal means to collect payment that is due. This may include billing you via certified mail, charging your credit/debit card, use of collections services, and notifying the Credit Bureau.
